The Department of Health Nurse will be at SODA on Thursday 8/18/2022 to administer vaccines for students 11 years and older. They will offer COVID, Tdap, MCV4, and HPV vaccines. You must register online for COVID vaccines,

Please go to Nurse office Documents from the main menu of website to get permission forms or have student pick one up from the nurse or office. Student must also have a completed form signed by parents. If you want your student to have a vaccine other than COVID vaccine please call the school nurse to ensure we have scheduled your student for the vaccines of your choice.
